Please arrive at your first appointment 15 minutes early to allow time for registration.   You will need to bring a photo ID as well as your health insurance card.  We have two entrances to our facility. The Main Entrance and the Walk-In Center Entrance. All co-payments will be collected at this time.

• All patients arriving for an appointment should enter through the main entrance (except for podiatry appointments, pediatric appointments, walk-in visits, and all appointments after 5 p.m. - use the walk-in center entrance).

For your first appointment, please bring all medications you are currently taking so we can record the names and dosage.
